New X- ray shows two levels lysis L5/S1, L3/L4,Intact discal spaces Normal alignment,Pedicles are intact • Examination of Lower back:General Inspection: Normal gait Palpation: No tenderness,SLR 90,Motor +5,DTR +2 ROM: full ROM Final Diagnosis:M43.0 Spondlolysis

لديك كما هو في التقرير الام في الظهر تمتد للطرف السفلي الايمن وخشونة بين الخامسة القطنية والاؤلى العجزية وبين الثالثة والرابعة القطنية , الانعكاسات العصبية سايمة , القوة العضلية كاملة , يمكن رفع الاطراف السفلية بمقدار تسعين درجة بمعنى لايوجد انزلاقات غضروفية, المشي سليم . حالتك تستدعي فقط العلاج الطبيعي وتقوية عضلات الظهر لحماية الغضاريف من الجفاف

ماذا يوجد في هذا التقرير the above pt was seenin my OPD today with severe back pain with L leg radiation for thw last 6 months following an RTA on the 29th Nov.2009. He was treated conservatively for the last number of months with no great benefit. On examination he is in significant pain and discomfort with limited ROM and positive SLR on the L leg. MRI scan shows a large L4-5 disc prolapse on L nerve root. He will requre further intervention for his condition.

يشرح هذا التقرير حالة المريض حيث يذكر ان المريض راجع العيادات الخارجية يشكو من الم في الظهر يمتد الى الساق اليسرى منذ 6 شهور على اثر حادث سير عولج المريض تحفظيا خلال الفترة السابقة بدون وجود بوادر للتحسن بالفحص السريري الحالي لا زال المريض يشكو من من الالم مع وجود محدودية في حركة الساق اليمنى اظهرت صورة الرنين المغناطيسي وجود انضغاط على جذر العصب المغذي للساق اليسرى يقع بين الفقرتين القطنيتين الرابعة والخامسة يستحسن اجراء المزيد من التقييم لحالة المريض لمعرفة الطريقة الامثل للعلاج

حابه اعرف نتيجة الاشعه (Radiology Result: CT Angiography Lower Extremity Bilat Reason for exam: bluish discoloration and coldness of the left leg and foot, previous ortho surgeries in the foot FINDINGS: The lower abdominal aorta, both common iliac, external iliac, femoral and popliteal artery are patent with adequate calibre and enhancement. No aneurysmal dilatation. No focal narrowing noted. Left leg: The inferior popliteal arteries (ATA, PTA and PA) showing adequate enhancement and can be traced downward to the ankle and foot. Right leg: The inferior popliteal arteries (ATA, PTA) showing faint enhancement however it can be traced downward with difficulty to the ankle and foot with opacification of the dorsalis pedis. The peroneal artery showing faint opacification proximally however cannot be traced in the lower leg. ما نتيجة هذه الصورة؟ Needle emg of both deltoid muscle showed no sopntenous activity bilaterally but large poyphasic mups of that of the right side.Ncs and f wave of right and lift median and ulnar and recording from apb and adm showed: Normal conduction velocity , latency, amplitude but delayed f response of right ulnar nerv"....Conclusion:Ncs study of the above mentioned nerves and muscles showed evidence of mild right C5-C7 proximal rediculopathy.

اعتلال في الجذور العصبية الخامس والسادس العنقيين من الجهة اليمنى وعصب المرفق من الجهة ايمنى , قد تحتاج صورة رنين مغناطيسي للعنق ومراجعة جراح اعصاب

عملت اشاعه على يدي اليمنه وبدي اطمن على نتيجه venous doppler examination of the right upper limb showed no evidence of any any acute or chronic thrombosis in the major deep and superficial veins

الواضح ليس لديك اي عائق في الاوردة السطحية او العميقة

ما هو التشخيص للأشعة على القدم Arterial color coded duplex scanning of both lower limbs revealed: Patent iliac arteries with normal Doppler waveform. Patent femoropopliteal and infrapopliteal arteries with diffuse atherosclerosis with no evidence of significant stenosis or obstruction and with normal triphasic Doppler waveform Venous Color Duplex scanning of both lower limbs revealed: 1.No evidence of deep or superficial venous thrombosis 2 No calf venous thrombosis. 3. No evidence of venous reflux could be detected in the superficial or deep venous system with competent saphenofemoral and saphenopopliteal junctions Conclusions: Diffuse atherosclerosis of the arterial tree of both lower limbs. Normal venous duplex study of both lower limbs.

التشخيص يقوم به الطبيب الذي طلب الصورة اعلاه , اليك خلاصة التقرير : لا يوجد علامات جلطات دم عميقة او تخثر في الاوردة ,لا خثرات في الساق , الاوردة سليمة , تصلب في شرايين الاطراف السفلية دون اغلاق , ينصح استشارة طبيب القلب والشرايين وعدم الاجتهاد في العلاج دون رأي الاخصائي
